Description

Responsibilities

*Conduct comprehensive assessments of patients with renal conditions, including physical examinations and review of medical history.

*Administer dialysis treatments and monitor patients during dialysis sessions, ensuring optimal therapy outcomes and patient comfort.

*Manage and assess vascular access sites, including arteriovenous fistulas and grafts, for dialysis patients.

*Monitor patients' vital signs, fluid status, and laboratory values, adjusting treatment plans as necessary to maintain physiological stability.

*Educate patients and their families about renal disease, treatment options, medication management, and lifestyle modifications to promote self-care and adherence to treatment regimens.

*Collaborate with nephrologists, social workers, and other healthcare professionals to develop comprehensive care plans tailored to individual patient needs.

*Document patient assessments, interventions, and outcomes accurately and timely in electronic medical records.

*Participate in quality improvement initiatives and adhere to regulatory standards and guidelines governing renal care.

Qualifications

*Diploma in Nursing from an accredited nursing program.

*Current licensure as a Registered Nurse (RN) with the Malaysian Nursing Board.

*Excellent communication and interpersonal skills with a patient-centered approach.

*Ability to work collaboratively in a dynamic healthcare environment.

*Commitment to continuous professional development and evidence-based practice in nephrology nursing.
